Key Responsibilities\n

• Conduct thorough psychosocial assessments of clients, incorporating emotional, mental, and social factors to, understand their needs better.

• Develop individualized care plans in collaboration with clients and their families, reflecting their unique goals and preferences.

• Provide counseling and emotional support during significant life transitions such as health changes, loss of independence, or bereavement.

• Act as a strong advocate for client rights, facilitating access to community resources that enhance their quality of life.

• Work closely with a multidisciplinary team, which includes healthcare professionals, to ensure seamless coordination of services.

• Regularly visit clients (virtually) to monitor their well-being, address living condition concerns, and ensure compliance with care plans.
